Initial Assessment

We start with a real conversation — not a form you fill out. One of our coordinators comes to your home, looks around, asks questions, and listens to what you're dealing with. This usually takes about an hour, maybe longer if you want to discuss details.

We look at things like:

  • Current health conditions and how they affect daily life
  • What tasks feel challenging right now
  • Your home layout and any mobility concerns
  • Family involvement and who's available to help

Personalized Care Planning

After that initial meeting, we develop a support plan. This isn't set in stone — it's a starting point that we adjust based on how things actually go.

We match you with team members whose experience and personality fit your situation. If you need someone who speaks Taiwanese or understands specific cultural preferences, we make that happen.

Regular Check-ins

Things change. Health improves, or sometimes it doesn't. New challenges come up. We schedule regular reviews to see how the support is working and whether we need to adjust anything. You're not locked into a plan that stops making sense.
